Key Responsibilities\n

• Conduct thorough assessments of patients to identify their unique mental health needs and collaboratively develop personalized treatment plans.

• Utilize evidence-based therapeutic techniques, including cognitive-behavioral therapy (CBT) and solution-focused brief therapy, tailored to individual client situations.

• Facilitate virtual support and psychoeducational groups for patients and families, providing a safe space for discussion and community connection.

• Work closely with medical professionals, including physicians and social workers, to ensure an integrated approach to patient care, particularly regarding mental health management within hospice settings.

• Provide crisis intervention and emergency support for patients experiencing acute psychological distress or related symptoms.
\n
• Educate patients and families on mental health topics, treatment methods, and available resources, empowering them to take an active role in their care.

• Maintain precise and confidential electronic records in compliance with all legal and ethical guidelines.
\n
• Participate in team meetings to share insights, collaborate on patient cases, and engage in peer consultations to enhance professional practice.

• Stay current with new research and trends in mental health care, particularly as they relate to hospice patients and end-of-life issues.

Patient Population / Client Focus\n

In this role, you will primarily support patients and families dealing with the emotional and psychological complexities associated with terminal illnesses and end-of-life care. Our client focus emphasizes empathy, dignity, and respect, striving to improve the quality of life during some of life’s most challenging moments. Your expertise will be crucial in helping clients navigate grief, anxiety, and depression, ensuring they feel heard and valued during their treatment.

Qualifications\n

• A Master’s or Doctoral degree in Social Work, Psychology, Marriage and Family Therapy, or Mental Health Counseling from an accredited institution.

• Current licensure as an LCSW, LPC, or LMFT in New Jersey.

• Deep understanding of therapeutic methodologies and mental health disorders, especially those relevant to hospice care.

• Demonstrated experience in developing and implementing effective treatment plans for diverse populations.

• Capable of facilitating both individual and group therapy sessions with clients of varying backgrounds.

• Outstanding verbal and written communication skills that foster trust and rapport.

• Strong ethical judgment and the ability to maintain professional boundaries.

• Familiarity with electronic health records (EHR) systems and various technology platforms.

Preferred Experience\n

• At least two years of direct experience working in mental health settings, preferably in hospice or palliative care environments.

Compensation and Benefits\n
