Work Environment

You will work onsite at our facility, delivering home care services directly to our clients' residences. Key Responsibilities

• Conduct in-depth evaluations of patients' physical abilities and challenges, developing tailored treatment plans that align with their personal rehabilitation goals.
• Deliver hands-on therapy services, including therapeutic exercises, manual therapy, and various modalities aimed at enhancing patient function and mobility.
• Monitor patient progress meticulously, documenting outcomes to adapt treatment strategies and ensure optimal recovery.
• Collaborate closely with interdisciplinary teams, including physicians, occupational therapists, and speech therapists, to coordinate comprehensive patient care strategies.
• Educate patients and their families on therapeutic techniques, home exercise programs, and methods for fall prevention.
• Implement and oversee fall prevention programs and safety measures, crucial for maintaining the wellbeing of clients living in assisted settings.
• Stay updated with current advancements in physical therapy practices and participate in ongoing professional development activities.
• Maintain adherence to ethical and professional standards, ensuring confidentiality and exemplary care practices.

Patient Population / Client Focus

Your primary focus will be on geriatric clients, offering much-needed services that respect their unique background and needs. This includes modifying treatment approaches to meet varying levels of physical ability and incorporating their personal histories into therapy sessions. Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution is required.
• Possession of an active state license to practice as a Physical Therapist.

Compensation and Benefits

The salary for this position ranges from $100,000 to $120,000 annually, commensurate with experience and qualifications.
