Work Environment

You will serve clients in their homes, witnessing firsthand the diverse and dynamic settings in which therapies take place. Our home care model allows for a convenient approach to therapy, enabling flexibility and personalization in treatment plans. Your daily schedule will typically span from Monday to Friday, from 2 PM to 8 PM, creating a conducive work-life balance while allowing for meaningful engagement with patients.

Key Responsibilities

• Conduct thorough patient assessments to determine functional limitations and establish individualized care plans in collaboration with both the patient and their healthcare team.
• Implement therapeutic interventions that promote recovery, using evidence-based practices that respect patient preferences and goals, focusing on rehabilitation and mobility enhancement.
• Provide direct physical therapy services, including skilled therapeutic care such as exercises and manual therapy techniques aimed at restoring function and reducing pain.
• Educate patients, families, and caregivers about therapeutic exercises, treatments, and strategies to facilitate ongoing recovery beyond the sessions.
• Regularly evaluate and document patient progress, adjusting treatment plans as necessary to foster continuous improvement in therapy outcomes.
• Manage a caseload effectively, prioritizing tasks and coordinating treatment plans with multidisciplinary team members to ensure seamless patient care.
• Engage in professional development and stay current with rehabilitation practices to provide the highest level of clinical expertise.

Patient Population / Client Focus

In this position, you will work with a variety of patient demographics, including adults recovering from surgeries and injuries, as well as pediatric clients. Your efforts will contribute to improved health across diverse conditions and challenges, including orthopedic rehab, neurological impairments, and gradual recovery from chronic illnesses. Compassionate care is key, as you will be addressing concerns with sensitivity and understanding while building trusted relationships with patients and their families.

Qualifications

• A current license to practice physical therapy in the state of Virginia is required.
• A Doctor of Physical Therapy (DPT) degree is preferable.
• Certification in Basic Cardiac Life Support (BCLS) is needed within the first 30 days of employment.
• While new graduates are encouraged to apply, at least 2 years of clinical experience is preferred.

Preferred Experience

• Experience in home health settings is advantageous and will enhance your ability to adapt treatment plans effectively.
• Familiarity with patient management software and electronic health records (EHR) systems is preferred to facilitate documentation and communication with care teams.

Compensation and Benefits
