Nealy Fitness Rehab Center is actively seeking a skilled and compassionate Home Care Physical Therapist to join our dedicated team in Carteret, New Jersey. Work Environment

This position involves onsite work within the clients' homes, where you will deliver therapy in a comfortable and familiar environment for each patient. Our home care setting allows for personalized interactions and direct assessments of clients' daily living challenges, offering a real-time and hands-on approach to rehabilitation. Working in the field means flexibility and variety in your day-to-day routine as you travel to various locations throughout Carteret and the surrounding areas. Our dedicated support team is always available to assist you with administrative and resource needs, allowing you to focus primarily on patient care.

Key Responsibilities

• Direct and oversee the implementation of personalized physical therapy treatments aimed at preventing disability and improving muscle function.
• Conduct comprehensive evaluations of clients' functional levels through a variety of diagnostic tests and assessments of muscle, nerve, joint, and functional abilities.
• Design and maintain individualized care plans in consultation with interdisciplinary team members, ensuring to update as necessary based on client progress.
• Execute manual therapeutic exercises and instruct clients in non-manual therapeutic activities beneficial for their rehabilitation.
• Administer various treatments, including traction and massage, with skilled application of physical agents as prescribed.
• Evaluate and recommend prosthetic and orthotic devices, ensuring they are fitted and adjusted properly to meet individual needs.
• Document all aspects of patient care including initial evaluations, progress notes, and discharge summaries in timely accordance with company policy.
• Facilitate communication among clients, families, and interdisciplinary team members to ensure cohesive care delivery.
• Mentor and supervise Physical Therapy Assistants to ensure the highest standards of care are upheld during their support services.
• Participate in interdisciplinary team case conferences for collaborative care planning and evaluation.

Our clients are individuals with varying physical disabilities and illnesses that require rehabilitation services to improve independence and quality of life. You will encounter a diverse population, including elderly clients and individuals recovering from surgeries, strokes, or debilitating conditions, all of whom rely on your expertise to enhance their functional abilities and promote better health outcomes.

Qualifications

• A Doctorate in Physical Therapy from an accredited program is required.
• Must possess current PT licensure in good standing within the state of practice.
• Proficient with clinical evaluation skills and diagnostic criteria relevant to pain and functional mobility.
• Current CPR certification and a valid driver's license along with automobile insurance.
• Demonstrated commitment to patient-centered care and clinical excellence.
• Strong interpersonal and communication skills, capable of engaging clients and their families effectively.

Preferred Experience

• A minimum of one year of experience working as a physical therapist in an acute or home care setting is preferred.
• Experience working within an interdisciplinary team is highly regarded.
• Familiarity with OASIS documentation and home health care regulations.

Compensation and Benefits
