Monadnock Care Solution Physical Therapy is seeking a dedicated Licensed Physical Therapist to join our team at our Sanford, Florida rehabilitation facility. This full-time, onsite position offers a competitive annual salary of $100,000 to $120,000, along with comprehensive benefits. Key Responsibilities

• Conduct initial evaluations by assessing patients referred by physicians, establishing baseline functioning and identifying specific rehabilitation needs.
• Develop and implement personalized treatment programs tailored to each patient's goals, ensuring adherence to established protocols and evidence-based practices.
• Utilize various therapeutic techniques to treat conditions across specialties—including orthopedic, pediatric, neurologic, and sports medicine—adjusting approaches as necessary based on patient progress.
• Deliver skilled therapeutic interventions both in-clinic and, when necessary, at patients' homes, aligning closely with physician-prescribed care plans.
• Maintain meticulous treatment documentation and progress notes, ensuring compliance with legal and ethical standards while tracking patient improvement.
• Educate and empower patients and their caregivers through effective communication and training, promoting self-management and adherence to rehabilitation protocols.
• Engage with multidisciplinary teams to maximize patient outcomes, leveraging the expertise of colleagues and sharing insights as a means of collective improvement.
• Meet established utilization goals and contribute to the overall operational effectiveness of the facility.

Patient Population / Client Focus

As a part of our comprehensive care model, you will engage with a diverse patient population ranging from children to seniors, each with unique physical therapy needs. Our clients include athletes recovering from sports injuries, individuals recovering from surgery, and patients experiencing chronic pain or neurological conditions. Your role will be essential in restoring mobility and enhancing quality of life for every individual you treat.

Qualifications

• Bachelor's degree in Physical Therapy from a CAPTE or APTA-accredited program.
• A valid physical therapy license in the state of Florida.
• Basic Life Support (BLS) certification from the American Heart Association.
• Exceptional written and verbal communication skills to facilitate effective patient interactions.
• Strong interpersonal skills to build meaningful relationships with patients and coworkers.
• Proficiency in using electronic health record (EHR) systems and other relevant software programs.

Preferred Experience

• A Doctorate of Physical Therapy (DPT) from an accredited institution is highly preferred.
• Two years of clinical experience in a physical therapy setting, ideally with exposure to various specialties.
• Current CPR certification.

Compensation and Benefits
