This role is pivotal in our interdisciplinary chronic pain functional restoration program within a remote hospice center. Key Responsibilities\n

• Conduct thorough assessments of patients' emotional and behavioral needs through interviews, direct observation, and standardized measurement tools to design individualized treatment plans.

• Utilize a range of therapeutic modalities, including cognitive-behavioral therapy, mindfulness, and supportive therapy, to address issues such as anxiety, depression, grief, and lifestyle adjustments associated with chronic illness.

• Facilitate group therapy sessions and workshops focused on pain management strategies and coping techniques, enhancing patients' skills for better quality of life.

• Provide expert consultation and emotional support to interdisciplinary team members in a collaborative environment to ensure integrated care that respects psychological perspectives.

• Maintain detailed and accurate medical documentation in compliance with HIPAA and state regulations, ensuring the continuity and quality of patient care.

Patient Population / Client Focus\n

You will primarily work with individuals suffering from chronic pain conditions, end-of-life issues, and associated psychological challenges, often exacerbated by the social isolation that can accompany their diagnoses. Your role is essential in helping these patients navigate their emotional landscapes, improve their coping skills, and restore their quality of life through empathetic and evidence-based interventions.

Qualifications\n

• A valid and active LCSW license issued by the State of California.

• A Master’s degree in Social Work from an accredited institution.

• A minimum of 2 years of post-graduate experience in a clinical setting, preferably with a focus on chronic illness or pain management.

Compensation and Benefits\n
