Key Responsibilities\n

• Conduct ongoing assessments to evaluate and meet the diverse needs of patients, employing evidence-based practices to inform treatment plans.

• Facilitate effective communication between supervisors, direct care staff, and family members to coordinate and monitor an individual’s progress.
\n
• Schedule appointments, home visits, and observations, maintaining a proactive approach to monitoring each client’s journey within the program.
\n
• Coordinate and execute home visits for in-home observations and data collection, ensuring comprehensive evaluations of patient circumstances.
\n
• Implement program plans effectively and ensure that therapeutic interventions align with the established goals for each client.
\n
• Participate in interdisciplinary team meetings to discuss individual treatment strategies and enhance collaborative care.
\n
• Prepare concise monthly and quarterly progress notes to document client developments and treatment efficacy.
\n
• Organize and execute training sessions for staff, ensuring everyone is well-equipped to implement the program plan and maintain accurate documentation.

• Provide supportive follow-up interviews and observations, ensuring continuity of care and addressing ongoing patient needs.
\n
• Maintain comprehensive records that ensure critical information is readily accessible to authorized personnel and stakeholders.

• Serve as a liaison between the public, mental health community, and Primary Counseling Center, actively working towards shared development goals.

• Assist in the creation and enhancement of training materials aimed at improving therapeutic interventions and program effectiveness.

Qualifications\n

• A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.

• Must hold active and unrestricted licensure as a Professional Counselor (LPC) in Pennsylvania.

• Strong background or specialization in family therapy, couples therapy, trauma, or substance misuse is highly preferred.
