Work Environment

Your work will primarily take place onsite within the patient's home environment, providing you with a unique opportunity to build rapport with clients in their comfort zones. Key Responsibilities

• Conduct comprehensive evaluations to assess patients' needs and develop individualized treatment plans based on their medical history and physician recommendations.
• Implement evidence-based therapeutic interventions designed to improve patients' physical function, mobility, and overall quality of life.
• Utilize a variety of treatment modalities, including manual therapy techniques, therapeutic exercises, and patient education, tailored to each patient's specific conditions.
• Maintain meticulous documentation within our electronic health record (EHR) system, ensuring compliance with organizational policies and regulatory standards.
• Collaborate closely with interdisciplinary teams, including physicians, occupational therapists, and nursing staff, to ensure holistic care and maximize patient outcomes.
• Provide ongoing education and support to patients and their families, guiding them on techniques and strategies to facilitate recovery and prevent re-injury.
• Set and meet utilization goals, contributing to the operational success of the team while prioritizing patient satisfaction.

Patient Population / Client Focus

In this role, you will work with a diverse array of patient populations including those recovering from orthopedic surgeries, neurological disorders, pediatric patients, and sports-related injuries. Your ability to relate to and empathize with your patients will play a crucial role in their rehabilitation journey.

Qualifications

• Bachelor's degree from an APTA or CAPTE-accredited program in Physical Therapy.
• A valid Physical Therapy state license in New York.
• Basic Life Support Certification from the American Heart Association.
• Strong communication skills, both written and verbal, to foster effective interactions with patients, families, and colleagues.
• Exceptional organizational skills and proficiency in using computer systems for record keeping and documentation.
• A commitment to building strong therapeutic relationships with patients to help them achieve their functional goals.

Preferred Experience

• Prior experience in a home care setting is highly desirable.

Compensation and Benefits
