Key Responsibilities\n

• Conduct initial assessments to understand patients' current mental health status and emotional needs.

• Develop individualized treatment plans that align with patients' goals and values, regularly updating them based on progress and feedback.

• Provide therapeutic interventions through virtual appointments for individuals facing life-limiting illnesses, grief, and trauma.

• Facilitate supportive relationships with patients’ families, offering guidance on coping strategies and emotional support as needed.

• Maintain accurate and confidential patient records using Simple Practice, ensuring compliance with all regulatory requirements.

• Engage in continuous professional development and participate in counselor support groups to enhance your practice.

Patient Population / Client Focus\n

In this role, you will primarily serve terminally ill patients and their families, requiring a deep understanding of the psychological and emotional challenges surrounding end-of-life care. You will address issues such as grief, anxiety, depression, and adjust to loss, providing essential support during one of the most challenging times in their lives.

Qualifications\n

• Active licensure as a mental health clinician (LMHC, LCSW, or LMFT).

• Bachelor's degree in Counseling, Marriage/Family Therapy, Social Work, or a related field is required.

• Demonstrated skills in treatment planning and mental health counseling.

• Excellent communication skills, both verbal and written, to effectively interact with patients and families remotely.

• Competence in using telehealth platforms and technology to conduct virtual appointments.
