Work Environment

This position is onsite and part of our Home Care facility, which provides personalized therapy services within clients' homes. Working in home environments allows for a unique perspective on patient challenges and enhances the quality of care you deliver. Flexibility in your schedule is a vital component of this role, with required weekly hours totaling 40 and occasional weekend availability as needed to accommodate patient care.

Key Responsibilities

• Conduct thorough evaluations of patients to assess physical function and limitations, developing individualized treatment plans based on findings.
• Implement treatment protocols and engage patients in their therapy, ensuring they understand procedures and encouraging active participation in their recovery.
• Document patient progress and treatment results accurately in accordance with all professional standards and regulatory requirements, including submitting charges for services rendered.
• Work collaboratively with an interdisciplinary team to coordinate patient care, discussing treatment plans and progress with physicians and family members regularly.
• Participate actively in community outreach initiatives and quality improvement projects to enhance our service offerings and maintain high standards within the organization.
• Oversee and mentor support staff, fostering a productive team environment to improve overall patient care.
• Stay current with industry advancements and maintain professional licensure through ongoing education and training.

Qualifications

• Bachelor's Degree in Physical Therapy is mandatory.
• A valid state license in Physical Therapy is required to practice in Virginia.
• Basic Life Support (BLS) certification is essential for all staff members.
• Proficiency in written and verbal communication, as well as computer skills, is expected for effective documentation and teamwork.

Preferred Experience

• Experience in a home health setting or working with diverse patient populations is preferred but not mandatory.
