Key Responsibilities\n

• Conduct thorough assessments to identify patients' needs and formulate individualized treatment plans that promote their mental health journey.

• Maintain open communication with supervisors, direct care staff, and families regarding individuals’ progress, ensuring a holistic approach to each client’s treatment.

• Generate and share a schedule of appointments, home visits, and observations with supervisors, ensuring alignment on therapeutic goals and client care.

• Coordinate in-home visits to conduct observations, chart reviews, and staff interviews, enhancing the comprehensive evaluation of patient needs.

• Ensure effective implementation of care plans, monitoring progress to make adjustments as necessary for the best outcomes.

• Participate actively in Interdisciplinary Team Meetings to discuss patient care strategies and collaborate on treatment innovations.

• Document monthly and quarterly progress notes that reflect clients' advancements and outline necessary adjustments to their care plans.

• Facilitate training sessions to ensure staff are proficient in implementing program plans and utilizing documentation tools effectively.

• Provide ongoing support, including follow-up interviews and observations, to ensure clients receive the care they need post-discharge.

• Maintain organized records that are easily accessible to team members and relevant stakeholders, prioritizing confidentiality and privacy.

• Serve as a vital liaison between the center and the mental health community, advocating for resources and support to enhance program development.

• Assist in crafting procedural training materials aimed at improving program quality and enhancing service delivery for clients.

Qualifications\n
