Key Responsibilities

• Conduct thorough evaluations, including comprehensive reviews of patient medical histories, assessments of physical capabilities, and the formulation of individualized treatment plans that address specific needs.
• Engage patients and their families by explaining evaluation findings and collaborating on realistic goal-setting, ensuring that patient participation is at the forefront of the care process.
• Implement treatment protocols tailored to each patient's unique presentation and modified as their conditions evolve, using best practices and staying current with changes in the healthcare landscape.
• Collaborate with other healthcare professionals to coordinate care, documenting any changes in a patient's condition that necessitate adjustments to their treatment plans or referrals for additional medical consultation.
• Ensure timely and accurate documentation of patient care activities, including clear initial evaluations, daily progress notes, and discharge summaries that reflect a comprehensive treatment narrative.
• Demonstrate effective time management to meet productivity expectations and maintain an organized schedule, participating actively in team meetings and community outreach initiatives.
• Contribute to the implementation of department goals by adhering to all regulatory and accreditation standards, while actively participating in the continuous improvement of care delivery and patient access initiatives.

Patient Population / Client Focus

You will serve a diverse population of patients who are recovering from various physical impairments, including post-surgical rehabilitation, sports injuries, neurological conditions, and general mobility challenges. Our team's goal is to empower patients through personalized care that not only addresses their physical limitations but also fosters their emotional and psychological resilience.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution is mandatory.
• A valid state license for physical therapy is required, along with certification in Basic Life Support (BLS) through the American Heart Association.

Preferred Experience

Compensation and Benefits
