Position Overview

Empower Concierge Rehab and PT is seeking a dedicated and licensed Outpatient Physical Therapist to join our innovative home care team in Estero, Florida. This full-time position will involve direct patient care in a supportive environment, focusing on rehabilitation and recovery for patients in the comfort of their homes. The therapist will engage with clients to evaluate their needs, devise tailored treatment strategies, and guide them through their rehabilitation journeys.

Work Environment

This position is based onsite, primarily visiting patients in their respective homes throughout Estero and surrounding areas. Key Responsibilities

• Conduct comprehensive evaluations for each patient, assessing their physical abilities, limitations, and personal rehabilitation goals.
• Design and implement personalized treatment plans that consider the unique circumstances and lifestyles of our diverse patient population.
• Educate patients and their caregivers on effective exercises, self-management techniques, and preventative strategies to enhance recovery.
• Collaborate closely with referring physicians, physical therapy assistants, and other health professionals to ensure cohesive, multidisciplinary care.
• Maintain accurate documentation including patient progress notes, treatment plans, and outcomes, adhering to regulatory standards.
• Create a motivating and empathetic atmosphere that encourages patients and fosters a positive mindset toward their recovery.

Patient Population / Client Focus

The therapist will work with a diverse patient demographic, including individuals recovering from surgical procedures, managing chronic pain, and undergoing rehabilitation for sports-related injuries. Our focus is on empowering patients through compassionate care, improving their quality of life, and helping them regain independence in daily activities.

Qualifications

• A valid and active Physical Therapy license in the state of Florida (or eligibility for licensure).
• A Bachelor's degree in Physical Therapy or a Doctorate in Physical Therapy from an accredited program.
• Exceptional communication, organizational, and interpersonal skills to effectively engage with patients and collaborate with healthcare professionals.
• A strong passion for patient care and a commitment to ongoing professional development.

Preferred Experience

• Minimum of 2 years experience in an outpatient clinical setting; however, recent graduates are encouraged to apply, as mentorship will be provided.
• Experience in home health care is a plus.
• A proven track record of developing effective treatment plans and achieving positive patient outcomes.
