Key Responsibilities\n

• Perform thorough assessments and screenings to evaluate the mental health needs of hospice patients, taking into account their unique emotions and psychological stressors during end-of-life care.

• Provide individualized short-term counseling sessions and effective crisis intervention services, addressing issues such as grief, anxiety, and depression.

• Facilitate supportive therapy groups and workshops focusing on topics like coping strategies, mindfulness, and family communication for patients and their caregivers.

• Build collaborative relationships with interdisciplinary teams, including doctors, nurses, and social workers, to enhance treatment plans tailored to each patient’s evolving needs.

• Engage in community outreach and preventive measures, such as mental health awareness initiatives, to foster understanding and support for hospice mental health care.

• Record and maintain accurate, confidential patient records, ensuring compliance with HIPAA regulations and ethical guidelines.

Patient Population / Client Focus\n

Our patient population primarily consists of individuals in hospice care, along with their families, who are navigating the complex emotional and psychological challenges associated with terminal illnesses. This role requires a deep understanding of the specific mental health issues these patients face, including grief, loss, and existential concerns.

Qualifications\n

• Possession of an active state license as a Licensed Independent Clinical Social Worker (LICSW), Licensed Mental Health Counselor (LMHC), Licensed Marriage and Family Therapist (LMFT), or Doctor of Psychology (PsyD).

• A Bachelor’s degree in Social Work, Psychology, Counseling, or a related field is required; a Master’s or doctoral degree is preferred.
