Key Responsibilities\n

• Conduct thorough mental health assessments for patients under hospice care, focusing on emotional and psychological needs specific to end-of-life issues.

• Develop and implement tailored treatment plans that acknowledge the unique circumstances and desires of patients and their families.

• Provide individual and family counseling sessions, utilizing evidence-based therapeutic techniques tailored to the end-of-life context.

• Employ crisis intervention strategies as necessary, ensuring that patients and their loved ones receive immediate and effective support during times of distress.

• Collaborate with hospice caregivers, medical teams, and family members to create a cohesive support system that fosters emotional healing and acceptance.

• Educate patients and families about the mental health impact of grief, loss, and terminal illness, providing resources and tools for coping.

• Maintain comprehensive documentation in compliance with legal, ethical, and organizational standards while utilizing electronic health records (EHR) systems for efficient patient tracking.

Patient Population / Client Focus\n

Your work will predominantly involve patients in hospice care, which includes individuals facing terminal illnesses and their families. You will work with clients experiencing a range of mental health challenges, including anxiety, depression, grief, and trauma. This role is crucial in providing emotional support, facilitating discussions about end-of-life choices, and promoting holistic well-being.

Qualifications\n

• Current and valid licensure as a Licensed Clinical Social Worker (LCSW), Licensed Professional Counselor (LPC), Licensed Marriage and Family Therapist (LMFT), or Doctor of Psychology (PsyD).

• Bachelor’s degree in a relevant field is required; however, a Master's degree or doctoral degree in Social Work, Mental Health Counseling, or Psychology is preferred.

• A minimum of two years of post-licensure experience is expected.

• Comprehensive knowledge of mental health disorders, trauma-informed practices, and evidence-based treatment modalities.

• Strong skills in conducting assessments, diagnosing mental health conditions, and effective treatment planning.

Preferred Experience\n

Experience in hospice or palliative care settings is highly desirable. Familiarity with grief counseling and the specific emotional challenges faced by patients and their families is a significant plus. Demonstrated ability to provide both individual and family therapy, as well as experience in crisis management in emotionally charged situations, will make you an ideal fit.

Compensation and Benefits\n

The salary for this role ranges from $60,000 to $104,000 per year, commensurate with experience and qualifications.
