Work Environment

This role is entirely onsite, requiring travel within the community as you provide personalized physical therapy directly in the homes of our clients. You will work within a supportive team environment, collaborating closely with fellow healthcare professionals to address and meet the unique needs of each patient.

Key Responsibilities

• Direct the development and implementation of individualized physical therapy treatment plans to prevent disability, alleviate pain, and restore or improve muscle function, ensuring alignment with the prescribed plans of care.
• Evaluate clients' functional abilities through diagnostic assessments, developing tailored interventions that consider both physical and environmental factors to enhance recovery.
• Supervise and mentor physical therapy assistants to ensure they execute treatment protocols effectively and adhere to best practices.
• Conduct home assessments to make recommendations for modifications or supportive devices that enhance patient independence.
• Administer manual and therapeutic exercises, alongside teaching clients and their families about effective non-manual exercises and daily functionality.
• Maintain comprehensive clinical documentation including evaluations, treatment progress notes, and discharge summaries, adhering to OASIS guidelines.
• Participate actively in interdisciplinary team meetings to strategize and adapt client care plans based on observed progress and feedback from the care team.
• Instruct clients and their families on the correct usage and maintenance of therapeutic equipment, ensuring they are confident in their ongoing therapy plans.

Patient Population / Client Focus

You will work with a diverse range of patients from varying backgrounds, typically including individuals recovering from surgery, patients with chronic illness, and older adults requiring rehabilitation support. The focus will always remain on fostering independence and improving the overall quality of life for each client. Your contributions will significantly impact their health outcomes and daily living capabilities.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited program.
• Active, unencumbered Physical Therapist license in New Jersey.
• Excellence in manual therapy techniques and the ability to implement successful personalized exercise programs.
• Proficiency in electronic medical records software, particularly Web PT, for effective documentation.
• Comprehensive knowledge of current physical therapy practices supported by evidence-based research.

Preferred Experience

• Prior experience in home health care settings is highly favorable.
• Demonstrated ability to work collaboratively within an interdisciplinary team.
• Experience in utilizing advanced therapeutic modalities and techniques will be considered a plus.

Compensation and Benefits

This full-time position offers a competitive salary ranging from USD 100,000 to 120,000 annually, complemented by a comprehensive benefits package that includes:
