Key Responsibilities\n

As a Licensed Mental Health Counselor, your key responsibilities will include but are not limited to:

• Delivering educational and goal-oriented counseling to patients who are pursuing transplant options, guiding them through the emotional and logistical complexities of this journey.

• Maintaining up-to-date knowledge of local vocational and educational rehabilitation programs to assist patients who wish to enter or re-enter the workforce or pursue further education.

• Evaluating patients' understanding of advance directives and providing necessary resources and support for discussions with their healthcare teams and families.

• Conducting thorough psychosocial assessments that identify strengths and challenges impacting patient rehabilitation and treatment outcomes as part of the comprehensive care team.

Patient Population / Client Focus\n

This role primarily focuses on supporting patients with complex emotional and psychological needs, including those facing chronic health conditions such as renal failure. Qualifications\n

• A valid Social Work License as per state regulations (LMHC, LMFT, LICSW).
