Key Responsibilities\n

• Act as a liaison between clients, families, and outpatient providers, ensuring seamless communication and comprehensive care coordination.

• Engage with insurance companies for pre-certification purposes and manage utilization effectively, advocating for clients' needs.

• Plan and participate in discharge and aftercare strategies tailored to each client’s specific circumstances and care requirements.
\n
• Provide therapeutic meal support, guiding clients in developing a healthy relationship with food while respecting cultural preferences.

• Offer empathetic and affirming care, considering each client's unique intersecting identities to foster a safe and supportive therapeutic environment.

• Document clinical notes accurately in our electronic health record system, maintaining compliance with all regulatory standards.

Patient Population / Client Focus\n

This role primarily focuses on older adults, but also includes a diverse client base experiencing various mental health concerns. You will work closely with individuals facing challenges such as anxiety, depression, and trauma, employing therapeutic techniques to promote healing and resilience. • A Master’s degree in Social Work from an accredited institution is required.

Compensation and Benefits\n
