Key Responsibilities\n

• Conduct thorough documentation of services provided on the same day for individuals at risk of crisis, ensuring continuity of care.

• Complete critical incident reporting within 24 hours when necessary, maintaining compliance with organizational policies.
\n
• Participate in after-hours phone availability on an alternating basis, providing essential care coordination support.
\n
• Proactively engage with clients to prevent crisis situations, utilizing effective interventions and therapeutic techniques.
\n
• Create and maintain comprehensive crisis alerts that ensure coordinated efforts with county crisis services.
\n
• Facilitate transitions for hospital or inpatient services, working closely with hospital staff to support smooth discharge processes.
\n
• Utilize crisis de-escalation skills and act as Clinician of the Day to provide milieu support in high-stress situations.
\n
• Complete risk assessments and develop safety plans for clients, ensuring their ongoing safety and well-being.

Patient Population / Client Focus\n

As a Licensed Mental Health Therapist at Clear Mindcare, you will work with a diverse population of clients who may experience various mental health challenges, including anxiety, depression, trauma, and other psychological disorders. Qualifications\n

• An independent mental health license in the state of Oregon (e.g., LCSW, LMFT, LPC) is required.
