Key Responsibilities

• Manage comprehensive therapeutic care for a designated group of patients, coordinating closely with physicians and multidisciplinary healthcare teams.
• Conduct thorough patient assessments to determine individual therapy needs and develop customized care plans that prioritize patient engagement and goal achievement.
• Deliver hands-on physical therapy interventions and education, ensuring interventions align with physicians' treatment plans and ethical standards.
• Evaluate and interpret therapeutic outcomes to enhance rehabilitation efforts, making necessary adjustments to treatment strategies when required.
• Instruct patients, families, and caregivers in therapeutic exercises, self-administered therapies, and the effective use of assistive devices.
• Monitor and prioritize patient caseloads, adjusting staffing needs and coordinating with colleagues for seamless patient transitions and hand-offs.

Patient Population / Client Focus

In this role, you will engage with a diverse patient population, including individuals recovering from surgery, trauma, or chronic conditions. You will play a vital role in facilitating rehabilitation for both adult and pediatric patients, ensuring each individual receives tailored care based on their specific health needs. Your contributions will significantly impact their recovery journey as you help restore mobility, strength, and overall well-being.

Qualifications

• A valid license in Physical Therapy (state-specific regulations apply).
• A Doctor of Physical Therapy (DPT) degree from an accredited institution.
• Basic Cardiac Life Support (BCLS) certification, or the ability to obtain it within 30 days of employment.

Preferred Experience

While no prior experience is required, candidates with previous internships or clinical rotation experience in a rehabilitation setting will be given preference. Familiarity with electronic health record systems and rehabilitation therapy methodologies will be advantageous.
