Work Environment

This position is specifically tailored for an onsite, full-time role in a home care setting, allowing our Physical Therapists to work closely with patients in their own environments. This approach enables a more tailored, personalized rehabilitation process that considers the patient's unique surroundings and lifestyle. Your work schedule will be Monday through Friday, from 2 PM to 8 PM, allowing you to provide care during optimal hours for patient convenience.

Key Responsibilities

• Deliver individualized, high-quality patient-centered care by managing the entire process of therapeutic intervention for a designated patient group, in collaboration with physicians and multidisciplinary care teams.
• Conduct thorough assessments of patient needs and develop customized, evidence-based care plans focusing on rehabilitation goals and patient engagement.
• Provide or oversee the administration of physical therapy treatments throughout the continuum of care, ensuring therapies are aligned with the prescribed care plan.
• Utilize your clinical knowledge to evaluate therapeutic and functional status, contributing to comprehensive treatment strategies.
• Administer effective and purposeful therapeutic interventions at the patient's home, employing techniques that align with physician directives and lead to significant improvement in patient functional abilities.
• Educate patients and their families on therapeutic exercises, the proper use of therapeutic appliances, and exercise regimens to be practiced outside of therapy sessions to enhance recovery.
• Prioritize caseload management, performing daily assessments of staffing needs, and coordinating patient assignments for optimal care delivery.

Patient Population / Client Focus

Your primary focus will involve working with patients recovering from orthopedic surgeries and conditions, which may include joint replacements, fractures, and sports injuries. This role requires compassion, patience, and skill as you support patients from various backgrounds with differing needs, all aiming to enhance their mobility and quality of life.

Qualifications

• Active Physical Therapy License in California.
• Doctor of Physical Therapy (DPT) degree from an accredited institution.
• Basic Cardiac Life Support (BCLS) certification, or the capability to obtain this certification within 30 days of starting.
• No prior experience is necessary, making this an excellent entry point for new graduates passionate about orthopedic rehabilitation.

Preferred Experience

• While experience is not required, candidates who have completed internships or clinical rotations in orthopedic settings may have an advantage.
• Familiarity with home health care settings and patient assessment tools is beneficial.

Compensation and Benefits
