Position Overview

Vitality Care Clinic is seeking a dedicated and compassionate Physical Therapist (PT) to join our team full-time at our rehabilitation center located in Ontario, California. Key Responsibilities

• Conduct comprehensive Physical Therapy evaluations for children and adults, assessing their physical abilities and limitations.
• Develop and implement individualized treatment plans that are evidence-based and align with patients' specific rehabilitation goals.
• Regularly update and modify treatment plans based on ongoing assessments and feedback from patients, ensuring progress is tracked through measurable outcomes.
• Document all patient interactions, assessments, and treatment plans in accordance with the company's electronic health record system, maintaining high standards of accuracy and timeliness.
• Educate patients and their families on rehabilitation processes, ensuring they understand treatment plans and potential challenges they may encounter.
• Meet established utilization goals and contribute to the clinic's broader business objectives, promoting a culture of high-quality care and efficiency.
• Collaborate with interdisciplinary team members to provide comprehensive care for patients receiving multiple therapeutic services.
• Actively promote and embody the core values of the clinic, advocating for best practices in patient care and community wellness.

Patient Population / Client Focus

You will engage with a diverse patient population, including children and adults with a wide range of physical impairments, neurological conditions, and orthopedic injuries. Qualifications

• Must possess a Master's degree or Doctor of Physical Therapy (DPT) from an accredited institution.
• Current and unrestricted licensure from the California Board of Physical Therapy.
• Willingness to provide therapy both in-person at the clinic and via telehealth for appropriate cases.
• Demonstrated desire to work with a diverse patient demographic, exhibiting cultural sensitivity and adaptability.
• Proficient in clinical assessment and adaptation of therapeutic techniques based on individual patient needs.
• Strong communication and interpersonal skills, fostering teamwork and collaboration within a multidisciplinary setting.

Preferred Experience

• Experience working in a rehabilitation center or similar healthcare setting.
• Prior involvement in patient education and family counseling initiatives is highly beneficial.
• Knowledge of current trends in physical therapy practices, including teletherapy methodologies.

Compensation and Benefits

The salary for this position ranges from USD 100,000 to 120,000 annually, commensurate with experience and qualifications. We offer a comprehensive benefits package, including:
