Key Responsibilities

• Perform comprehensive Physical Therapy evaluations to assess the physical function and mobility of each patient.
• Devise and implement personalized treatment plans based on individual patient assessments and progress towards therapeutic goals.
• Regularly monitor and reassess patient outcomes, documenting clinical findings in compliance with the clinic's electronic health record system.
• Engage in effective communication with patients and their families, providing education on treatment plans, recovery expectations, and strategies to overcome challenges associated with their rehabilitation.
• Collaborate with fellow healthcare professionals within the clinic to deliver integrative care for clients requiring multiple services, ensuring a cohesive treatment approach.
• Maintain accurate and timely documentation including SOAP notes and progress reports to facilitate high-quality patient care.
• Work towards meeting and exceeding minimum utilization targets and clinic objectives, contributing to the overall success and growth of Vitality Care Clinic.

Patient Population / Client Focus

Qualifications

• A Master's degree or Doctor of Physical Therapy (DPT) from an accredited institution is required.
• Current, unrestricted licensure to practice Physical Therapy in the state of California obtained from the Board of Physical Therapy.
• Demonstrated willingness and ability to provide both in-person and telehealth services, ensuring that all patients receive appropriate care regardless of their circumstances.

Compensation and Benefits
