Peak Performance Physical Therapy is actively seeking a dedicated and skilled Orthopedic Physical Therapist to join our dynamic team in Rancho Cucamonga, California. Work Environment

The position is primarily onsite, allowing for direct interaction with patients as part of our Home Care facility. Our office is equipped with state-of-the-art tools and technology to ensure our therapists can offer the most effective treatments available. You will be joining a small, dedicated team comprised of two experienced physical therapists and one physical therapy assistant (PTA), all of whom are passionate about delivering high-quality patient care.

Key Responsibilities

• Conduct thorough evaluations of patients' physical conditions to understand their goals and needs.
• Develop individualized treatment plans focused on enhancing mobility, reducing pain, and improving quality of life.
• Manage a caseload of patients, ensuring each individual receives attention and care tailored to their specific circumstances.
• Implement evidence-based therapeutic interventions and rehabilitation strategies, using tools such as manual therapy, exercise prescriptions, and modalities.
• Document patient progress meticulously in our electronic medical records system, particularly WebPT, to maintain accurate treatment records and satisfy regulatory requirements.
• Provide education and guidance to patients regarding their rehabilitation process, promoting healthy lifestyle choices and preventing further injuries.
• Regularly assess treatment outcomes, make necessary adjustments, and communicate effectively with patients and their families.
• Collaborate with other healthcare professionals to ensure a comprehensive and coordinated approach to patient care.

Patient Population / Client Focus

Your primary focus will be on patients recovering from orthopedic surgeries, injuries, and chronic conditions requiring rehabilitation. This diverse patient population will provide you with the opportunity to apply your skills and creativity in various treatment settings. You will work closely with these patients, becoming an integral part of their recovery journey.

Qualifications

• A Master's or Doctorate degree in Physical Therapy (DPT) is required.
• A valid state license to practice as a Physical Therapist, with no history of board or disciplinary actions.
• Demonstrated proficiency in manual therapy techniques and developing effective exercise protocols tailored to individual patient needs.
• Experience utilizing EMR software, particularly WebPT, to facilitate streamlined documentation and patient management.

Preferred Experience

• A minimum of two years of experience in an orthopedic outpatient setting is preferred.
• A background in developing and implementing innovative wellness programs is an asset.
• Experience collaborating with multidisciplinary teams to enhance patient outcomes is advantageous.

Compensation and Benefits
