Heal Motion Care is seeking dedicated and passionate Physical Therapists to join our growing team in Ontario, California. Work Environment

This position is based in our outpatient office in Ontario, California, where a collaborative and supportive atmosphere thrives. Our facility is equipped with state-of-the-art tools and resources to assist in delivering quality patient care. The full-time work schedule is Monday through Friday from 8 AM to 3 PM, allowing for a balanced work-life dynamic while still meeting the therapeutic needs of our patients.

Key Responsibilities

• Conduct comprehensive Physical Therapy evaluations for clients, especially children, to assess their needs and establish individual treatment strategies.
• Develop and implement personalized treatment plans that align with the therapeutic goals for each patient while ensuring regular adjustments based on progress and clinical observation.
• Continuously reassess clinical signs and symptoms to evaluate treatment effectiveness, making necessary modifications to ensure optimal outcomes.
• Maintain accurate and timely documentation of patient progress, including SOAP notes, care plans, and progress summaries, utilizing our electronic record system.
• Educate patients and their families about the recovery process, setting realistic expectations and addressing any challenges they may encounter in a compassionate manner.
• Achieve minimum utilization goals and contribute to the organization's objectives, playing a key role in the operational success of our facility.
• Collaborate closely with other therapy disciplines within Heal Motion Care to provide integrated care for patients receiving multiple services.
• Embody and promote the core values of Heal Motion Care, acting as a brand ambassador in all interactions with patients, families, and colleagues.

Patient Population / Client Focus

This role focuses on a diverse patient population, including children and adults with varying physical abilities, backgrounds, and conditions. As a Physical Therapist at Heal Motion Care, you will engage with patients facing challenges ranging from post-injury rehabilitation to chronic pain management. Your ability to adapt therapy approaches based on individual cultural and linguistic needs is paramount to enhancing patient engagement and treatment effectiveness.

Qualifications

• A Bachelor's Degree in Physical Therapy or a related field from an accredited institution.
• Current and unrestricted licensure by the Board of Physical Therapy in California.
• A strong willingness to provide in-person and telehealth evaluations and treatments, embracing flexibility in service delivery according to patient needs.
• Demonstrated commitment to working with diverse patients across all age groups, displaying sensitivity and adaptability to various cultural contexts.
• Ability to thrive in a dynamic environment, showcasing agility and responsiveness to changing situations and patient demands.
• A team-oriented mindset with a strong focus on customer service, willing to collaborate with colleagues to enhance patient care.

Preferred Experience

• Prior experience in an outpatient setting, particularly working with pediatric and adolescent populations, is a significant advantage.
• Familiarity with electronic health records and documentation practices is preferred to streamline patient management processes.
• Experience in using telehealth platforms for patient consultations and therapy sessions.
