Regular case conferences and meetings will facilitate coordinated efforts in care planning and progress monitoring.

Key Responsibilities\n

• Develop and implement individualized support plans aimed at addressing clients' unique needs and challenges.

• Conduct assessments and interviews with clients and their families to evaluate their emotional, social, and developmental needs.

• Maintain detailed and accurate documentation of client interactions and care progress, adhering to confidentiality and ethical standards.

• Participate in case conferences, presenting findings and providing recommendations to enhance service delivery and support integration.

• Serve as a liaison, referring clients to community resources that facilitate their recovery journey and address any external barriers to care.

Compensation and Benefits\n
