Work Environment

This position is onsite, working primarily in patients' homes across the Saint Petersburg area. Key Responsibilities

As a Physical Therapist within our home care team, your responsibilities will include:

• Directing physical therapy treatments aimed at preventing disability, relieving pain, and enhancing muscle function in accordance with individualized treatment plans.
• Conducting thorough evaluations of patients' functional capacities through diagnostic tests and assessments to inform treatment strategies.
• Collaborating with physical therapy assistants, providing supervision and guidance to ensure effective implementation of care plans.
• Assessing home environments to make appropriate modifications that promote patient independence and safety during rehabilitation.
• Administering a variety of therapeutic treatments, including manual therapy, exercises, and application of physical agents like traction and massage.
• Fitting and adjusting orthotic and prosthetic devices, and recommending modifications as needed.
• Engaging in ongoing communication with physicians and case managers to report observations, document progress, and adapt treatment plans based on patient response.
• Maintaining comprehensive clinical records and documentation in accordance with regulatory and organizational standards, ensuring that all reports align with OASIS requirements.
• Providing education and support to patients and families regarding their therapy regimens, equipment usage, and lifestyle adjustments that promote recovery.

Patient Population / Client Focus

In this role, you will serve a diverse patient population, including elderly individuals recovering from surgery, patients with chronic illness, or those with mobility challenges. Qualifications

Candidates must meet the following qualifications:

• Possess a Bachelor's Degree from an accredited physical therapy program.

• Hold a current PT license in good standing in the state of Florida.
• Maintain current CPR certification.
• Have a valid driver's license, reliable transportation, and automobile insurance to travel to patient homes.
