Belares’ Family Care Center in Waldorf, Maryland, is excited to announce an opening for a dedicated and experienced Family Nurse Practitioner (FNP) to join our innovative healthcare team. In this full-time position, you will play a pivotal role in providing comprehensive, patient-centered care through our unique concierge medicine model and house call services. Your expertise will ensure our patients receive top-notch medical attention tailored to their individual needs.

Work Environment

This position is based onsite in Waldorf, Maryland, where you will engage primarily with patients in their homes or other community settings. Our full-time FNPs work collaboratively with a team of healthcare professionals in a supportive environment that fosters communication, learning, and growth. Typical working hours are from 8:00 a.m. to 5:00 p.m., with some flexibility for evening clinics and participation in weekend health events.

Key Responsibilities

As a Family Nurse Practitioner at Belares’ Family Care Center, you will be responsible for a range of critical tasks, including:

• Conducting thorough health assessments and screenings for individuals and families throughout the community.
• Diagnosing and managing acute and chronic medical conditions, and providing preventive care and health education.
• Ordering and interpreting diagnostic tests, and utilizing evidence-based practices to develop treatment plans.
• Collaborating with community organizations to create and implement impactful health education programs.
• Advocating for patients' rights and access to healthcare, ensuring culturally sensitive care in every interaction.
• Coordinating referrals and follow-up care to ensure seamless continuity of care for all patients.
• Keeping detailed, accurate medical records in compliance with healthcare regulations.
• Participating actively in community outreach events, including health fairs, to raise awareness about health issues and provide necessary screenings.
• Staying informed about community health trends and continuously expanding your knowledge of relevant medical advancements.

Patient Population / Client Focus

Your work will focus on diverse patient populations, including families and individuals from various backgrounds, emphasizing preventive health and holistic care. You will play a crucial role in enhancing the physical, social, and emotional well-being of patients and their families, ensuring that every visit is tailored to meet their specific healthcare needs. By doing so, you will help to bridge gaps in healthcare access and improve overall community health outcomes.

Qualifications

• A current certification as a Nurse Practitioner, and an active state nursing license.
• A Master's degree in Nursing from an accredited institution.
• A minimum of 3-5 years of clinical experience, particularly in community health or public health settings, is required.
• Strong interpersonal and communication skills, with the ability to effectively work with and relate to diverse populations.

Preferred Experience

While not mandatory, experience in concierge medicine or home health services can be an advantage. Candidates who have worked with underserved populations or in collaborative care environments will find their skills to be a great fit in our practice.

Compensation and Benefits

We offer a competitive annual salary ranging from $180,000 to $200,000, based on experience and qualifications. Additionally, our comprehensive benefits package includes:
