Key Responsibilities\n

• Conduct thorough, ongoing assessments to accurately identify clients' needs and develop effective treatment plans.

• Facilitate communication among supervisors, direct care staff, families, and other stakeholders to ensure coordinated care and track individual progress.

• Provide a detailed schedule of appointments, home visits, and progress monitoring to supervisors, ensuring all activities are documented and aligned with patient care goals.

• Coordinate essential home visits that involve in-home observations, comprehensive chart reviews, and staff interviews to gather diverse insights on client needs.

• Ensure the effective implementation of the treatment program plans, actively participating in interdisciplinary team meetings to discuss client status and adjust approaches as necessary.

• Maintain accurate records and documentation to ensure transparency and easy access to information for all involved parties.

• Develop and organize training sessions to enhance staff competencies in implementing treatment plans and documentation requirements.

• Provide ongoing support through follow-up interviews and observations, reinforcing positive outcomes for clients.

• Act as a liaison with the surrounding mental health community to foster collaborative relationships and support for client development goals.

Qualifications\n
