Key Responsibilities\n

• Work closely with your treatment team to design and implement coordinated care strategies tailored to the unique needs of clients and their families.

• Engage in effective communication with internal programs and external partners, such as educational institutions and community organizations, fostering a collaborative environment.

• Maintain compliance with agency billing requirements and ensure that all services meet program specifications.

• Participate in on-call rotations, which may include obtaining LPS designation and responding to after-hours client needs, demonstrating flexibility and commitment.

• Attend and contribute to staff meetings, providing insights that aid in program development and enhance staff training initiatives.

• Demonstrate reliability by maintaining punctuality and consistent attendance in your remote role.

• Conduct case management activities encompassing the assessment of client needs, identifying therapeutic interventions, and linking clients to appropriate resources both within the community and through clinical support.

Compensation and Benefits\n
