Key Responsibilities\n

• Conduct thorough evaluations and assessments to identify the psychosocial needs of seniors and their families.

• Participate in the development and implementation of Individualized Family Service Plans (IFSPs) that reflect client goals.

• Monitor and report on client outcomes, facilitating continuous improvement in care delivery.

• Provide emotional and practical support to families, helping them adjust to the challenges of caregiving and health management.

• Ensure compliance with all relevant federal and state regulations and agency policies.

Patient Population / Client Focus\n

Qualifications\n

• A Master's degree in Social Work (MSW) or a related field is required.

• An active Licensed Clinical Social Worker (LCSW) credential in the state of Texas.

Compensation and Benefits\n
