Key Responsibilities\n

• Coordinate comprehensive clinical handoffs for at-risk patients, including appropriate referrals to other services when necessary.

• Build and maintain an accessible support network, providing counsel and resource information both for patients and their families.

Qualifications\n

Compensation and Benefits\n

HEARD Emotional Wellness Solution, Inc. offers a competitive salary range of $50,000 to $80,000 per year, commensurate with experience and qualifications. Additional benefits include:
