This remote position focuses on providing essential mental health support to a diverse client population, including seniors navigating the challenges of addiction and mental health. Work Environment\n

This position is classified as full-time and predominantly remote, allowing for flexibility and a comfortable working environment. You will be primarily working with a detox center clientele, collaborating closely with multidisciplinary teams, including nurses and healthcare professionals, to provide comprehensive care. While standard hours will typically be Monday to Friday from 8:00 AM to 4:30 PM, flexibility in scheduling is essential to accommodate client visits and ensure ongoing support. Occasionally, you may need to participate in evening or weekend hours to meet urgent client needs. On-call support may also be required during critical times.

Key Responsibilities\n

• Conduct comprehensive assessments of seniors’ psychosocial needs, taking into account emotional, mental, and social factors impacting their well-being.

• Develop and implement individualized care plans in collaboration with clients and their families, centered around specific goals and preferences.

• Provide counseling and emotional support to clients, assisting them in navigating life transitions and overcoming challenges related to addiction.

• Advocate for clients’ rights and ensure access to community resources, helping clients obtain necessary support and services.
\n
• Work in tandem with cross-disciplinary teams to enhance care delivery and optimize client outcomes.

• Perform regular visits to clients’ residences to assess well-being, evaluate living conditions, and address any health or safety concerns.

• Facilitate effective communication within families, helping to resolve conflicts that may arise surrounding care, and improve overall family dynamics.

• Educate clients and families on community resources and support groups designed to foster social engagement and improve quality of life.
\n
• Maintain accurate and comprehensive documentation, including client assessments, care plans, and progress notes, adhering to ethical and regulatory standards.

• Stay abreast of current practices, policies, and regulations in social work to ensure ongoing compliance and commitment to best practices.

Qualifications\n

• A Master's degree in Social Work from an accredited institution.

• Current and valid state licensure as a Clinical Social Worker (LCSW).

Preferred Experience\n

Experience working in a detox or substance abuse setting is highly desirable, as well as familiarity with community-based programs targeting addiction recovery. A background in geriatric care and knowledge of relevant legal and ethical guidelines surrounding social work practices would be considered an asset.

Compensation and Benefits\n
