Key Responsibilities\n

• Deliver therapeutic sessions through an intensive 8-week evidence-based program designed to enhance patient skills and resilience.

• Monitor and evaluate participant progress regularly, ensuring effective interventions with comprehensive follow-up communications and care coordination recommendations.

• Assist participants in accessing essential resources and facilitate the completion of consent and authorization forms to streamline care processes.

• Identify high-risk scenarios, implementing appropriate measures in line with company protocols to safeguard participant welfare.

• Engage actively in care team meetings, monthly team interactions, and Town Halls, contributing your insights and expertise to enhance team performance.

• Uphold the highest standards of confidentiality in accordance with HIPAA regulations, ensuring all practices meet compliance training requirements.

Qualifications\n

Applicants must possess one or more of the following credentials:

Compensation and Benefits\n

The compensation for this full-time position ranges from USD 115,000 to 130,000 annually, commensurate with experience and hours worked. Additional benefits include:
