Work Environment\n

This role is a full-time position allowing for remote work flexibility. Although the majority of the work can be done from home, occasional visits to clients may be necessary. Your work schedule will typically be Monday to Friday, from 8:00 AM to 4:30 PM, with flexibility required to accommodate client needs, including the potential for occasional evening or weekend hours.

Key Responsibilities\n

• Conduct comprehensive psychosocial assessments of seniors to evaluate their emotional, mental, and social needs.

• Collaborate closely with clients and their families to create personalized care plans, setting specific goals that align with their preferences.

• Provide individual and family counseling, offering emotional and practical support to help navigate challenging life transitions.

• Act as an advocate for clients, ensuring their rights are protected and they have access to necessary community resources and services.

• Work with interdisciplinary teams, including healthcare professionals, to enhance client care and coordination.

• Perform regular check-ins and visits to assess client conditions, ensuring safety and quality of living.

• Facilitate family communication, helping resolve conflicts that may arise during the caregiving process.

• Educate clients and families about available community resources, support groups, and programs to promote social engagement and enhance overall well-being.

• Document assessments, care plans, progress notes, and significant interactions accurately in the electronic health record (EHR) system.

• Stay current with best practices in social work and adhere to ethical standards in all client interactions.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.

• Current state licensure as a Licensed Clinical Social Worker (LCSW) is essential.

• At least two years of professional experience in social work, ideally within a hospice, geriatric, or community-based setting.

• Strong understanding of aging-related issues and familiarity with community resources available to seniors.

• Exceptional skills in assessment, counseling, and problem-solving, with the ability to empathize and connect with seniors and their families.

Compensation and Benefits\n

This position offers a competitive salary range of USD 60,000 to 100,000 yearly, commensurate with experience.
