Work Environment

This role is 100% onsite at patients' homes, requiring you to interact directly with patients in their familiar environments. Key Responsibilities

As a Physical Therapist at Holy Trinity Medical Centre, your responsibilities will include:

• Conducting comprehensive evaluations of patients referred by physicians, assessing their physical condition and identifying individualized treatment goals.
• Developing, implementing, and adapting treatment plans based on the patient's unique needs and the physician's directives.
• Delivering therapeutic interventions to a diverse patient population encompassing orthopedic, pediatric, neurologic, and sports medicine cases.
• Utilizing advanced therapeutic techniques and state-of-the-art equipment to facilitate recovery and restore function effectively.
• Providing education and training to patients and caregivers, empowering them with knowledge about their treatment plans and ongoing self-care strategies.
• Documenting patient progress meticulously using electronic medical records (EMR), ensuring compliance with regulatory and organizational standards.
• Collaborating closely with a multidisciplinary team to optimize patient outcomes and improve care delivery processes.
• Meeting established utilization goals while maintaining a focus on high-quality patient care.

Patient Population / Client Focus

You will primarily work with patients requiring home care services, including recovering surgical patients, those with chronic pain conditions, and individuals facing challenges related to mobility. Qualifications

To be considered for this role, candidates must possess:

• A Bachelor's degree from an accredited physical therapy program recognized by the APTA or CAPTE.
• State licensing in physical therapy, allowing you to practice legally within Florida.
• A Doctorate degree in Physical Therapy (DPT), which is preferred for this role.
• Basic Life Support (BLS) certification from the American Heart Association, ensuring readiness to respond to medical emergencies.
