Work Environment

This position is entirely onsite, allowing you to work directly in the homes of your patients, facilitating an intimate and focused therapeutic environment. You will collaborate closely with a multi-disciplinary team, which may include physicians, nurses, and other healthcare providers to ensure a cohesive treatment plan that respects the preferences and needs of each patient. Your role will be crucial in navigating challenges unique to home care, including conducting evaluations in diverse living conditions and adapting therapy plans accordingly.

Key Responsibilities

• Direct and implement personalized physical therapy treatments aimed at preventing disabilities, alleviating pain, and restoring muscle function based on individual patient assessments.
• Evaluate each patient's physical condition through various diagnostic tests to formulate effective therapy plans in accordance with physician guidelines.
• Oversee and mentor Physical Therapy Assistants to ensure compliance with therapeutic protocols and assist in patient-centered care.
• Conduct comprehensive evaluations of patients' home environments to identify potential barriers to rehabilitation and recommend modifications that promote independence.
• Administer an array of therapeutic techniques, including manual exercises, traction, and the application of physical agents, tailored to patient capabilities.
• Regularly update patient progress documentation, including initial evaluations, care plans, and discharge summaries, ensuring accurate clinical records for ongoing assessment and care adjustments.
• Actively participate in interdisciplinary team meetings to discuss treatment strategies and outcomes while engaging with patients and families to educate them about their therapy regimens.
• Maintain compliance with scheduling requirements and documentation protocols, ensuring timely submission of clinical and payroll records.
• Facilitate educational sessions for patients and their families regarding the use and maintenance of physical therapy equipment, reinforcing treatment goals in everyday life.

Patient Population / Client Focus

Your primary patient population will consist of individuals recovering from orthopedic surgeries, injuries, or chronic conditions that require specialty rehabilitation. You will focus on fostering recovery through personalized treatment, enhancing mobility, reducing pain, and improving overall quality of life. A patient-centered approach will guide your interactions, ensuring that your care is responsive to each patient's unique cultural, emotional, and physical needs.

Qualifications

• A Bachelor's Degree in Physical Therapy from an accredited institution.
• A minimum of one year's experience as a physical therapist, with experience in either acute care or home care environments.
• Current physical therapy licensure in good standing in Virginia.
• Current CPR certification to ensure readiness for emergencies.
• A valid driver's license and proof of automobile insurance, as travel to patients' homes will be necessary.

Preferred Experience

Compensation and Benefits

This position offers a competitive salary range of USD 100,000 to 120,000 annually, commensurate with experience.
