Key Responsibilities\n

• Conduct thorough assessments to evaluate patients' mental health status and safety, identifying potential risks and formulating appropriate interventions such as crisis management for issues like suicidal ideation or self-harm.

• Develop individualized treatment plans based on thorough assessments, collaborating with clients and their families to incorporate their insights and preferences.

• Facilitate psychoeducational sessions, process groups, and individual therapy sessions utilizing evidence-based methodologies that promote healing and personal growth.

• Collaborate effectively with a multidisciplinary team, including psychiatrists, social workers, and other mental health professionals, to ensure comprehensive care and successful discharge planning.

• Maintain accurate and timely documentation, including assessments and treatment progress notes, in compliance with organizational policies and healthcare regulations.

• Participate in ongoing treatment review meetings, contributing insights to enhance care plans and ensuring continuity of care post-discharge.

• Deliver post-discharge follow-ups to support clients in their recovery journey and assess their needs for further support.

Qualifications\n
