Work Environment

This role is based in a home care setting, where you will have the unique opportunity to deliver personalized therapy in the patients' own environments. Key Responsibilities

• Conduct comprehensive evaluations of patients in their homes to assess their physical abilities and rehabilitation needs.
• Develop, implement, and adapt individualized treatment plans that incorporate various therapeutic methods including exercises, manual therapy, and the use of specialized equipment to enhance mobility and alleviate pain.
• Regularly document patient progress and responses to treatment, ensuring clear communication with both the patient and the health care team.
• Identify when to reassess goals and treatment strategies based on patient recovery and progress, demonstrating adaptability in care plans.
• Assist patients in acquiring and using assistive devices to promote independence in self-care and mobility.
• Educate patients, their caregivers, and family members on physical therapy techniques and the proper use of assistive devices to empower them in their recovery journey.
• Engage in professional development by attending workshops, seminars, and participating in relevant professional organizations to stay updated with the latest in physical therapy practices.

Qualifications

• A Bachelor's Degree in Physical Therapy is required; candidates with a Doctorate Degree are preferred.
