Key Responsibilities

• Conduct thorough evaluations of patients referred by healthcare providers to develop personalized treatment plans.
• Utilize evidence-based methodologies to implement effective treatments for various conditions, including orthopedic, pediatric, neurologic, and sports-related issues.
• Deliver compassionate care in patients' homes, ensuring adherence to physicians' plans while adapting techniques to promote improvement in function and quality of life.
• Provide educational resources for patients and their caregivers to support home exercise programs and enhance long-term health outcomes.
• Demonstrate a strong commitment to customer service, ensuring a welcoming and supportive environment for patients and their families.
• Collaborate with interdisciplinary teams to align on treatment goals and improve patient outcomes through a collective approach.
• Maintain accurate documentation of treatment sessions, progress notes, and billing processes according to organizational standards.

Patient Population / Client Focus

This role focuses on a varied patient demographic, including individuals recovering from surgeries, managing chronic conditions, or overcoming injuries. Whether you're working with athletes returning to play or elderly clients needing support for daily living activities, you will play a pivotal role in enhancing independence and wellness in their daily lives.

Qualifications

• Graduate from an APTA or CAPTE-approved physical therapy program.
• Bachelor's degree in Physical Therapy is required; a Doctorate of Physical Therapy (DPT) is preferred.
• Current state licensure as a Physical Therapist in New Jersey.
• Certification in Basic Life Support from the American Heart Association.
• Strong oral and written communication skills to effectively interact with a diverse patient population.
• Proficient in computer and documentation systems relevant to patient care.

Preferred Experience

• A minimum of two years of clinical experience in physical therapy.
• Demonstrated expertise in diverse treatment settings, including home care and outpatient environments.
• Experience working with interdisciplinary teams and a strong desire to contribute positively to patient outcomes.
• Current CPR certification.
