Monadnock Care Solution Physical Therapy is seeking a dedicated and skilled Physical Therapist to join our team in Woodbridge, New Jersey. Work Environment

This role is primarily onsite, as you will be working directly with patients in their homes across Woodbridge and surrounding areas. Key Responsibilities

• Conduct thorough evaluations of patients referred by physicians, assessing their physical conditions and functional limitations to develop personalized treatment plans.
• Implement evidence-based therapeutic techniques and treatment modalities to address a range of specialties, including orthopedic, pediatric, neurologic, and sports medicine cases.
• Provide skilled, purposeful care in patients' homes, which may involve manual therapy, therapeutic exercises, and education on pain management techniques.
• Maintain effective communication with patients and their families to ensure understanding of the treatment process and expectations.
• Track patient progress against established goals, adapting treatment plans as necessary and documenting progress in compliance with state regulations.
• Work collaboratively with a multidisciplinary team, consisting of other healthcare professionals and support staff, to maximize patient outcomes through cohesive care delivery.
• Engage in continuous self-improvement by participating in training sessions and utilizing feedback to enhance service delivery.

Patient Population / Client Focus

You will primarily work with diverse patient populations, including individuals recovering from surgery, managing chronic pain, or in need of rehabilitation due to injury. Our holistic approach ensures that each treatment plan is tailored to individual needs, aiming for significant improvements in functionality and mobility. Qualifications

• A Bachelor's degree in Physical Therapy from an accredited program recognized by APTA or CAPTE.
• Active state license in Physical Therapy, along with a current certification in Basic Life Support (BLS) from the American Heart Association.
• Strong written and verbal communication skills, facilitating efficient exchanges of information with patients, families, and team members.
• Proficiency in using electronic health record systems and general computer applications to document patient care accurately.
• Ability to establish rapport with patients, showing empathy and understanding to support their emotional and physical healing processes.

Preferred Experience

• A Doctorate of Physical Therapy (DPT) is preferred but not mandatory.
• A minimum of two years of clinical experience in a relevant setting is highly desired.
• Previous experience in home health care or a community-based outpatient clinic will be an advantage.
• Current CPR certification and familiarity with evidence-based practice methodologies.
