Work Environment

This position is primarily onsite, allowing therapists to provide care in patients' homes, ensuring they receive the support they need in a comfortable setting. Our flexible scheduling allows you to choose hours that work best for your life while considering patient needs. Generally, therapists work Monday through Friday, enjoying weekends off to recharge.

Key Responsibilities

• Conduct thorough evaluations of patients in their home environments to assess their physical condition, mobility issues, and rehabilitation needs.
• Develop and implement individualized treatment plans tailored to each patient's specific recovery goals, utilizing various methods such as therapeutic exercises, manual therapy techniques, and mobility training.
• Monitor and document patient progress, ensuring all treatment responses are tracked accurately in our electronic health record system.
• Regularly reassess and modify treatment plans as needed based on ongoing evaluations, patient responses, and recovery progress.
• Provide guidance on assistive devices and recommend appropriate equipment for enhancing mobility and independence in daily activities.
• Deliver educational sessions to patients, caregivers, and family members, ensuring they understand the therapy process and how to support recovery at home.
• Engage in continuous professional development by attending educational workshops, subscribing to professional literature, and networking within the rehabilitation community.

Patient Population / Client Focus

The Home Care Physical Therapist will work with a diverse range of patients, including children and adults dealing with post-operative recovery, injuries, chronic pain, and various physical disabilities. Each therapist is expected to adapt their approach to meet the unique needs and challenges of each patient, ensuring a holistic approach to healing and rehabilitation.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited institution, with a valid state license in Physical Therapy.
• Exceptional verbal and written communication skills, essential for effective patient interactions and collaboration with healthcare teams.
• Proficiency in digital record-keeping and therapeutic technologies.
• A flexible and agile demeanor, enabling adaptation to the changing needs of patients.

Preferred Experience

• A minimum of 2 years of experience in physical therapy is required, ideally in home health or outpatient settings.
