Key Responsibilities

As a Nurse Practitioner within the CACC, your daily responsibilities will include:

• Obtaining detailed patient histories and conducting thorough physical examinations to assess cardiac health.
• Ordering and interpreting diagnostic tests, and diagnosing as well as managing both chronic and acute cardiac conditions.
• Formulating and implementing individualized care plans that focus on health maintenance, anticipatory guidance, and therapeutic interventions in accordance with established clinical protocols.
• Collaborating with physicians and fellow NPs to refine treatment plans based on patient progress, adjusting strategies as necessary to ensure optimal care outcomes.
• Providing education and counseling to patients and their caregivers regarding cardiac health, treatment options, and ongoing disease management.
• Coordinating hospital admissions and discharges, working alongside the inpatient team to ensure seamless transitions in care.
• Documenting all patient interactions within the electronic medical record (EMR) system in a timely and comprehensive manner.
• Participating in research protocols to enhance clinical practices and contribute to the advancement of cardiac care.
• Participating in quality improvement initiatives aimed at elevating patient outcomes within the CACC.
• Maintaining a safe and therapeutic environment congruent with our patient-centered care philosophy.

Patient Population / Client Focus

You will primarily focus on adult patients within the advanced Heart Failure, Pulmonary Arterial Hypertension, and post-Heart Transplant populations. Your expertise will be vital in addressing complex patient needs, overseeing those undergoing advanced therapies, and providing critical support for outpatient intravenous and inhaled medication management, as well as mechanical circulatory support. Understanding the unique challenges and requirements of this diverse group is essential for fostering effective patient relationships and facilitating optimal health outcomes.

Qualifications

To be considered for this rewarding role, candidates must possess:

• A Master’s degree in Nursing, with specific training as a Nurse Practitioner focused on adult care.
• A valid Nurse Practitioner license and Registered Professional Nurse (RN) license in New York State.
• NP board certification and current BCLS, ACLS, DEA, and NPI certifications.
• Proficiency in electronic medical records systems as well as familiarity with MS Word, MS PowerPoint, and MS Excel.

Preferred Experience

Ideal candidates will have:

• A minimum of 2 years of experience in a hospital environment, with a preference for those with direct experience in cardiology.
• A background in managing patients with complex cardiac conditions, particularly those requiring advanced therapies.

Compensation and Benefits
