Work Environment

This onsite position takes place primarily in patient homes throughout the Niagara Falls area. Key Responsibilities

• Conduct thorough assessments of patients’ health by gathering comprehensive medical histories, performing physical examinations, and ordering necessary diagnostic tests.
• Diagnose acute and chronic conditions, developing tailored treatment plans that address the specific needs and goals of each patient.
• Monitor patient outcomes by conducting follow-up visits and adjusting treatment plans as necessary to promote health and recovery.
• Provide education and counseling to patients and their families on disease prevention, health management, and necessary lifestyle changes.
• Document all patient interactions and treatment plans accurately in compliance with privacy and healthcare regulations.
• Collaborate with an interdisciplinary healthcare team, including physicians, nurses, therapists, and social workers, to ensure patient-centered care.
• Engage in continuous education through workshops, seminars, and conferences to stay informed about the latest medical advancements and best practices in nursing.

Qualifications

Compensation and Benefits

The Nurse Practitioner position offers a competitive salary between $150,000 to $160,000 annually, based on experience and caseload management.
