Key Responsibilities\n

• Conduct thorough and ongoing assessments to identify the mental health needs of your clients, tailoring treatment plans accordingly.

• Prepare and coordinate a detailed schedule of appointments, in-home visits, and monitoring activities to optimize service delivery.

• Engage in home visits to observe client conditions and conduct reviews, ensuring that care is comprehensive and tailored to individual needs.

• Implement and oversee program plans that support each client’s goals and objectives while tracking effectiveness and compliance.

• Participate actively in Interdisciplinary Team Meetings, contributing insights from your clinical assessments to enhance holistic care.

• Document monthly and quarterly progress notes that reflect changes in client conditions and adjust interventions as required.

• Organize and facilitate training for staff, ensuring that all team members are proficient in the implementation of client care plans.

• Provide continual support through follow-up interventions and maintain clear records for all client interactions.

• Act as a liaison with the public and mental health community, promoting understanding and collaboration in support of broader mental health initiatives.

Patient Population / Client Focus\n

Your role will involve working with a diverse client population, including both adults and youth facing varying degrees of mental health challenges. Qualifications\n

• A Master's or Doctoral degree in Counseling, Psychology, or a closely related field.

• Active and unrestricted licensure as a Clinical Professional Counselor (LCPC) in the state of Illinois.
