You will conduct sessions and consultations primarily through telehealth platforms, facilitating meetings with clients and their families while adjusting to their unique schedules. Although this is a remote position, you will maintain a strong connection with team members through regular virtual meetings and collaborative case discussions. It is vital to have the adaptability to conduct home visits as needed, ensuring thorough support for our clients.

Key Responsibilities\n

• Conduct comprehensive assessments of seniors' psychosocial needs, encompassing emotional, mental, and social dimensions to create a holistic understanding of each client.

• Collaborate with seniors and their families to create individualized care plans tailored to their specific goals and needs, promoting engagement and participation in their own care.

• Provide ongoing counseling, emotional support, and guidance to help seniors navigate the complexities of aging, chronic illness, or life transitions.

• Advocate for clients regarding their rights and access to vital community resources, ensuring they receive the necessary services tailored to their situations.

• Work within interdisciplinary teams comprising nurses, caregivers, and other healthcare professionals to coordinate comprehensive care strategies that enhance clients' overall well-being.

• Perform regular follow-up visits, either virtual or in-person, to monitor clients' well-being, living conditions, and address any evolving concerns or safety issues.

• Educate clients and families about community resources, support networks, and programs that encourage socialization and active engagement in the community.

• Resolve conflicts that may arise among family members, fostering effective communication and collaboration in the caregiving process.

• Maintain precise and thorough documentation of client assessments, care plans, progress notes, and all interactions in accordance with ethical and legal standards.

• Stay updated on best practices, policies, and legal regulations in social work, ensuring compliance with standards of quality care.

Your primary focus will be seniors within the Port Saint Lucie area, who often face an array of challenges such as chronic illnesses, mental health concerns, life transitions, and social isolation. Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.

• Current and valid state licensure as a Licensed Master Social Worker (LMSW) is essential.

• Minimum of two years of professional social work experience, preferably in a setting that serves elderly populations.

• Thorough knowledge of social work principles, issues related to aging, and available community resources in Florida.

• Exceptional interpersonal, assessment, and counseling skills, with the ability to empathize and connect with seniors and their families.

• Experience using Electronic Health Record (EHR) systems and proficiency in basic computer applications.
