Work Environment

The position is based onsite in the homes of our clients throughout the Saint Petersburg area, allowing for personalized interaction and care management. Key Responsibilities

• Develop and implement individualized physical therapy treatment plans aimed at preventing disability, relieving pain, and restoring muscle function, aligned with each patient's specific objectives.
• Supervise and mentor Physical Therapy Assistants to ensure effective execution of treatment plans while maintaining a high standard of care.
• Conduct thorough evaluations of clients' functional capabilities, employing diagnostic assessments to inform treatment strategies.
• Assess home environments and make recommendations that promote safety and independence for clients receiving care.
• Administer a variety of therapeutic techniques, including manual exercises, traction, and massage, along with physical modalities as prescribed.
• Evaluate, fit, and adjust orthotic and prosthetic devices, advising physicians on necessary modifications to enhance patient mobility.
• Document all clinical activities, including initial assessments, care plans, progress notes, and discharge summaries in accordance with OASIS standards.
• Actively participate in case conferences and team meetings, contributing to comprehensive care plans that address the holistic needs of our clients.
• Provide education and support to clients and their families regarding treatment methods and the use of therapeutic devices.
• Foster positive relationships with clients, families, and other professionals, ensuring collaborative care and clear communication of patient progress.
• Maintain accurate clinical records and timely documentation to comply with all regulatory requirements and company policies.

Patient Population / Client Focus

As a Home Care Physical Therapist, you will primarily work with orthopedic patients recovering from surgeries, injuries, or chronic conditions. The personal nature of home care allows you to connect deeply with patients, understanding their unique circumstances and tailoring your approach to meet their specific rehabilitation goals. Qualifications

• Graduate of an accredited physical therapy program with valid licensure to practice in Florida.
• A minimum of one year of experience as a Physical Therapist in a clinical or home care setting.
• Current CPR certification and a valid driver's license, along with automobile insurance.

Compensation and Benefits
