Key Responsibilities\n

• Assist primary care providers in recognizing and managing mental health issues, facilitating referrals when necessary.

• Conduct detailed patient intakes, focusing on diagnostic assessments and functional evaluations to inform treatment plans.

• Provide brief and focused interventions, using evidence-based practices to address acute mental health needs.

• Monitor patient progress through regular follow-ups, adjusting treatment approaches as necessary to ensure optimal outcomes.

• Develop and implement relapse prevention strategies, helping patients maintain stability in their mental health.

• Document patient progress and clinical information accurately in treatment charts, adhering to agency and FQHC guidelines.

• Promote health behavior changes among patients, offering guidance on lifestyle adjustments to enhance overall well-being.

• Collaborate with clinic management and other team members to evaluate and improve behavioral health services.

• Support patients in navigating their medical treatments, equipping them with strategies to manage medication side effects.

Qualifications\n

Compensation and Benefits\n

We offer a competitive salary range of USD 87,000 – 105,000 per year for this full-time position.
