Work Environment

Key Responsibilities

• Conduct thorough patient assessments through home visits, identifying health concerns, patient history, and personal goals.
• Diagnose and manage acute and chronic conditions, developing personalized care plans that reflect best practices in patient care.
• Implement evidence-based interventions and health promotion strategies, with a strong focus on disease prevention and patient education.
• Collaborate with interdisciplinary teams to ensure comprehensive care and effective communication regarding patient status and health outcomes.

Qualifications

• Bachelor's degree in Nursing from an accredited institution, along with a current Nurse Practitioner certification.

Compensation and Benefits
