Work Environment

This Physical Therapist role is primarily onsite, working with patients in a home care setting. Key Responsibilities

• Conduct thorough evaluations of patients' physical capabilities, assessing impairments, functional limitations, and any changes in health status due to injuries or illnesses.
• Develop and implement individualized treatment plans based on orthopedic physician orders, tailored to each patient's specific health goals and age-related needs.
• Document all patient interactions comprehensively and accurately, including evaluations, treatment plans, progress notes, and discharge summaries, using professional terminology.
• Supervise and provide guidance to Physical Therapist Assistants (PTAs) and support personnel, ensuring effective implementation of patient care strategies.
• Foster collaborative relationships with a multidisciplinary team including case managers, occupational therapists, and speech-language pathologists to coordinate patient interventions and monitor program effectiveness.
• Participate in regular team meetings to discuss patient progress, identify solutions to care-related challenges, and contribute to maintaining productivity standards and cultivating a positive work atmosphere.

Qualifications

Compensation and Benefits
